Sierra Leonean born Dr Radcliffe SoyinkaLisk has been part of the ‘Care of the Elderly Team,’ who have won the prestigious HSJ (Health Services Journal) Award dubbed – the Oscars of the health world!
According to the health Services Journal “The Care of the Elderly team won the award in the ‘Managing Long Term Conditions’ category for their Nursing Homes Project, aiming to improve healthcare for elderly nursing home residents and reduce emergency admissions to hospital. The project has already reduced emergency admissions from participating nursing homes by 40%.”
“Since 2010 the team has instigated monthly meetings between GPs, the hospital and nursing home staff to discuss individual residents and the best way to manage their care, including providing intravenous antibiotics and fluids in nursing homes where needed.”
One of the recipients DrKeefaiYeongsaid “This project highlights the importance of collaborative working between GPs, nursing home staff and hospital specialists in caring for the frail older person. It has resulted in more people being cared for successfully in what are effectively their own homes.”
Chief Executive Andrew Liles added: “I would like to congratulate our Elderly Care Consultants and the whole Team for this national recognition of their outstanding work in the Nursing Homes Project. It is great to see our staff working with colleagues in the community to help them deliver care in the best place for patients.”
